Dr. Suzanne Dibble, RN — VP of Health Care Services
Dr. Dibble is Adjunct Professor at the Institute for Health & Aging and Physiological Nursing. She received her doctoral education at the University of California, San Francisco. Dr. Dibble focuses on developing care models for those with chronic illnesses and terminal diseases, including breast cancer. She is dedicated to promoting culturally appropriate clinical care and is the founder and Co-Director of the Lesbian Health Research Center. She is a prolific writer who has many published books and articles in the lay and academic press. Her books include: Culture and Clinical Care (2005), You and Your Asthma (1980), and Culture and Nursing Care: A Pocket Guide (1996) which was chosen as one of the most valuable texts of 1996 by the American Journal of Nursing. Sue received her doctorate in Nursing from UCSF.
Elizabeth Landsverk, MD — Geriatric Physician
Dr. Landsverk, Founder of ElderConsult, serves the older individuals in her community in multiple roles. She makes house calls to address the spectrum of concerns facing frail elders up until their last day. She was recruited to teach UCSF Family Practice Residents in the academic year 2008-2009 and has served as the Medical Director of Vitas Hospice. In her consulting role, she advises patients and their care givers on issues related to aging, including maintaining strength and agility, screening for chronic diseases, identifying and living with memory problems, diagnosis and treatment of dementia and agitation, mood disorders, and pain. She teams with Primary Care Physicians on issues including medications, goals of care, family concerns, and strategies to help individuals maintain independence. Elizabeth got her MD from Stanford University, and trained at the University of Virginia, Harvard Medical School, and Mt. Sinai in NYC.
Allan Treadwell, MD — Internal Medicine and Geriatric Physician
Dr. Allan Treadwell practices Internal Medicine in San Francisco and Marin County, California. Dr. Treadwell graduated with an MD 19 years ago from The George Washington University School of Medicine and Health Sciences. Dr. Treadwell completed his residence in Internal Medicine at the University of California Davis Medical Center. Dr. Treadwell was honored with the Patients' Choice Award in 2008 and 2009. Currently, Dr. Treadwell shares his expertise among his practice at the UCSF Parnassus campus, his boutique medical practice that specializes in home-visits and emergencies, and Living Well members.
Jeanne Jennings — Registered Nurse
Jeanne Jennings RN has 14 years experience as a nurse serving in the San Francisco Bay Area. She brings with her a wealth of experience in Hospice and Palliative Care, as well as public health and Psychiatric Nursing. While volunteering she found serving with seniors the most rewarding by working to find ways to allow people to safely age in their own homes. Jeanne has completed training in Beginning and Gentle Yoga, and taught Yoga and Meditation classes for the past ten years. She leads weekly Yoga classes at the Integral Yoga Institute and community centers in the Bay Area. Jeanne will help further Living Well members' wellbeing as part of our Case Management Team.

Deborah Huelbig — Operations Coordinator

Deborah has more than 18 years of experience in business management and operations. Working for some of our major retail stores such as Target, Rite-Aid and TJ Maxx, Deborah has had experience in customer service and business management. While with these companies Deborah excelled in personnel management, scheduling overseeing payroll as well as other human resources, operations tasks and projects. In recent years, Deborah has chosen to apply her acquired skills in operations management to the field of eldercare. Her experience in scheduling, hiring, terminations, medical testing, record keeping and directly overseeing of clients' care have been key to smooth running client care and management. She is a long-term San Franciscan who earned a college degree in Business at Skyline College while working full time. At Living Well Deborah is a very enthusiastic and a loyal member of our operations team who takes to heart the care needs of each of our members. It is her passion to assist our growing aging population, helping find a better way for our aging population to age with dignity and grace safely in their own homes.

Elke Tekin — Business Development and Membership Services

Elke is a graduate of Notre Dame de Namur University in Belmont, California, where she studied both Economics and Finance. She was a Founder of AgeSong, Inc., an innovative development and management company of elder housing communities where she worked for over 10 years and which won the 2006 American Society on Aging - National Award for Excellence in the Field of Aging. Elke has over 20 years of management and marketing experience in the field of high tech and aging. She is licensed by the California Department of Social Services as a Residential Care Facility for the Elderly Administrator and is the Vice President of the Board of the San Francisco Residential Care Association. Elke is passionately involved in aging, health and longevity issues and partnered with Living Well to support the outreach and business development efforts. In her free time, you will find her out in nature with her son or at the family restaurant, Hayes & Kebab, a Mediterranean Grill in San Francisco.

Kiyoka Yukimoto, Psy.D. — Member Services and Wellness Assistant
Kiyoka received her doctorate in clinical psychology from the California School of Professional Psychology at Alliant International University in San Francisco. She wrote her dissertation on a study of homicide-suicide between older adults and their family caregivers in Japan. The majority of her clinical experience has been working with elders, dependent adults and their family caregivers in USA and Japan. She completed her practicum at the Institute on Aging in San Francisco and her pre-doctoral internship at the Pacific Institute and AgeSong communities providing psychotherapy, neuropsychological assessments and music therapy for elders; support groups for professional caregivers and brings all her compassion and experience to Living Well to further its mission of providing personalized and comprehensive care to its members.
Dr. Janet Meiselman — Geropsychologist and Wellness Faculty
Janet is a licensed clinical psychologist who specializes in providing mental health and wellness services to older adults, their families and caregivers. She completed her predoctoral clinical training in neuropsychology at Laguna Honda Hospital and her postdoctoral training at the Institute on Aging in San Francisco and Bay Area Psychological Services. Over the past seventeen years, she has worked with older adults, their families, and caregivers to assess and treat mental health challenges associated with the second half of life including late life depression, anxiety, complicated bereavement, adjustment disorders, substance abuse problems, and cognitive impairment.

Dr. Paul Lofholm, PharmD
Paul W. Lofholm received his Doctor of Pharmacy Degree from the University of California at San Francisco in 1964. He joined the Faculty at his Alma Mater in 1966. He is a Clinical Professor of Pharmacy. He is a community pharmacist, teacher, nursing home consultant and owner of Ross Valley Pharmacy in Larkspur, CA and Golden Gate Pharmacy in San Rafael, CA. He has received the Daniel B. Smith Award in 1972 presented by the American Pharmaceutical Association as the Outstanding Community Pharmacist of the Nation. He received the Alumnus of the Year Award from UCSF in 1976. The Albert E. Rossica Award was presented to him as the Preceptor of the Year in 1986. The Victor H. Morganroth, Jr. Memorial Award was presented in 1995 for outstanding contributions to improve patient care, pharmacy practice and education. The J. Leon Lascoff Award was presented for outstanding service to the profession in 1997. He was elected to the CPhA Hall of Fame in 2006 and he was the Pharmacist of the Year in 2007. He has developed an innovative practice severing several niches. The pharmacy is Medicare Certified for diabetes education. Pain and hormone replacement preparations are regularly compounded and we serve on the Hospice Pain Management team.
Dr. Courtney Jones, PharmD
Courtney Jones, PharmD earned her Doctorate of Pharmacy degree from Ferris State University in 2006. Currently she is the Pharmacist-in-Charge at Golden Gate Pharmacy. Dr. Jones has completed extensive training in pharmaceutical compounding from the Pharmaceutical Compounding Centers of America. She is certified to administer immunizations and has completed a certificate program in Community Aging, Assisted Living, and Long Term Care Pharmacy. She is a member of the Phi Lambda Sigma Leadership Society, American Pharmacists Association, National Community Pharmacy Association, California and Michigan Pharmacist Association, American College of Veterinary Pharmacy, American Society of Consultant Pharmacists, American Society of Health-Systems Pharmacists, and the Marin County Pharmaceutical Association.

Elizabeth Krivatsy, Attorney — Estate Planning and Elder Care
Elizabeth Krivatsy is an estate planning and elder law attorney based in Sausalito, California. She is passionate about helping people plan for the best possible future — drafting wills and trusts that reflect their values and protect their assets, preparing for personal care and financial management during times of incapacity, and choosing where and how they will live in retirement. Her work combines a lifetime of professional, personal, and volunteer experiences to help clients fully understand all of their options. Elizabeth often speaks to community and professional organizations about aging in place, planning for retirement living, preventing elder financial abuse, and making informed end-of-life decisions. She is a founding member of BNI Network 54, she has co-chaired the Sausalito Chamber of Commerce's Membership Committee for two years, and she provides pro bono legal services to vulnerable elders through Legal Aid of Marin and her church. Elizabeth lives in Sausalito with her husband Adam Krivatsy and a sweet Yellow Lab named Akayla, who sheds 12 months a year.

His responsibilities included serving as the primary point of contact to affluent and high net worth clients. He worked closely with a team of experienced specialists to offer financial planning, trust and estate services, investment management, brokerage, insurance and customized credit solutions. Prior to joining Wachovia in 2007, Lucas was a Vice President and Private Banker with Wells Fargo in San Francisco for 6 years where he held a similar position providing financial solutions to high net worth clients. Lucas graduated from the University of Southern California in 1993 with a B.S. in Business Administration. Lucas serves on the Board of Overseers for the Bay Area Chapter of Peer Health Exchange, a national non-profit organization that trains college students to teach a health curriculum in public high schools. Lucas lives in Mill Valley with his wife and 2 young children and is passionate about bringing his experience to Living Well members and families to plan well and age well.
Carolyn Rosenblatt, Esq. — Elder Law and Mediation
Ms. Rosenblatt is a Registered Nurse with ten years of experience in nursing, and twenty-seven years of experience as a lawyer, representing injured persons. She has been a mediator for three years, after representing many clients at mediation as their advocate. Her areas of expertise as a mediator include personal injury, elder abuse and neglect, and a variety of other subjects including defective products, medical negligence, parent-teacher-school conflicts and real estate disputes. She has considerable experience in working with elders, social services concerning them, and the families of elders. Ms. Rosenblatt is a passionate advocate for Living Well members' wellbeing.
